What Is a Fractional Business Manager – and Does Your Clinic Actually Need One?

Fractional Business Manager.

It sounds a little corporate, doesn’t it?

But the concept is actually really simple – and for growing clinics, it can make a lot of sense.

As your clinic grows, there usually comes a point where you realise that simply having someone helping you complete tasks isn’t enough anymore.

You’ve got ideas. You want to grow. You might want another practitioner, another treatment room, a second clinic or additional revenue streams.

But who’s actually making all of that happen?

That’s where a Fractional Business Manager comes in.

What is a Fractional Business Manager?

A Fractional Business Manager is an experienced business professional who works within your business on a part-time or fractional basis, rather than being employed as your full-time Business Manager.

Think of it as having access to the experience and input of a Business Manager without necessarily employing someone for 40 hours a week.

They work alongside you to help manage and improve the operational side of the business – overseeing projects, systems, processes, team operations, performance and growth plans.

And the important word there is manage.

They’re not simply waiting for you to send them a list of tasks.

They’re helping you work out what needs to happen in the first place – and then taking responsibility for helping to make it happen.

Why hire someone fractionally?

For many independent clinics, hiring a full-time Business Manager is a big commitment.

There’s the salary itself, but employing somebody also comes with the wider responsibilities and costs of employment – PAYE administration, employer National Insurance where applicable, pension obligations, holiday and sick leave, equipment and the ongoing responsibility of managing another employee.

And you might not actually need a Business Manager five days a week.

Fractional support gives you another option.

You can bring experienced management support into the business at the level you actually need.

For a growing clinic, that can mean accessing someone who works in business operations day in, day out and has experience supporting other businesses facing similar challenges – without immediately creating another full-time position.

It gives the clinic owner far more flexibility.

Does your clinic actually need a Fractional Business Manager?

Not every clinic does.

If you’re a solo practitioner, you’re happy with the size of your business and everything behind the scenes is manageable, you might not need business management support yet.

But things start to change when you’re trying to grow.

You might be:

  • Increasing your team or bringing in additional practitioners
  • Introducing new treatments, services or revenue streams
  • Trying to improve profitability
  • Opening another clinic or location
  • Improving the client journey
  • Changing or implementing new systems
  • Trying to understand your business numbers
  • Managing multiple projects at once
  • Finding that everything still needs your approval
  • Spending far too much of your week managing the business instead of leading it

These are no longer just individual admin tasks.

They’re business operations.

And somebody needs to take ownership of them.

What’s the difference between an assistant and a Fractional Business Manager?

This is an important distinction – particularly because at OSCO, we’ve provided both types of support.

A brilliant assistant can be invaluable to a business.

You delegate something. They complete it. The task disappears from your list and you get some time back.

But ultimately, you’re still managing the work.

You’re identifying what needs doing, deciding how it should be done, delegating it and often checking that it’s happened.

A Business Manager works differently.

Rather than asking:

“What would you like me to do?”

They’re much more likely to be asking:

“What are we trying to achieve – and what needs to happen to get us there?”

They can identify problems, suggest improvements, oversee implementation, coordinate people, manage projects and hold responsibility for moving things forward.

That’s a very different level of support.

Sometimes, delegating more isn’t the answer

This is something we’ve learnt first-hand through working behind the scenes with business owners.

You can delegate 20 tasks and absolutely get some time back.

But if you’re still the person who has to identify those 20 tasks every week, prioritise them, explain them, answer questions and decide what happens next…

You’re still the operational centre of the business.

And that’s where growing clinic owners can become stuck.

The problem isn’t necessarily that you need somebody to do more.

You might need somebody who can own more.

Imagine being able to discuss where you want the clinic to go and having someone work alongside you to turn that into an operational plan.

Someone who can say:

“Right, if that’s the goal, here’s what needs to happen next.”

And then actually help implement it.

That’s the difference.

What can a Fractional Business Manager actually do in a clinic?

The exact role will depend on your clinic and where you’re trying to go.

It could include reviewing and implementing your systems, improving workflows, overseeing projects, supporting team operations, tracking KPIs, improving your client journey, documenting processes, coordinating launches, reviewing what’s working operationally and helping implement your growth plans.

Maybe you know you need a new booking system but have no idea which one.

Maybe you want to introduce memberships but haven’t had the time to work out how they’ll operate.

Maybe you’ve hired more practitioners and suddenly communication, rotas, responsibilities and processes are becoming messy.

Or perhaps you’ve got ten ideas for growing the clinic and absolutely no capacity to implement any of them.

Those are the types of problems operational management can help solve.

The goal isn’t to remove you from your business

It’s to stop the business being completely dependent on you.

There’s a big difference.

As the owner, you should still have the vision.

You should still make the important decisions.

You should still know what’s happening.

But you shouldn’t necessarily have to be the person remembering, organising, implementing and overseeing every single thing.

A Fractional Business Manager sits alongside you and helps turn the things you want for the business into things that actually happen.

And ultimately, that’s what growing a business requires.

Not just more ideas.

Implementation.

So, does your clinic need one?

A good place to start is asking yourself:

Do I need another pair of hands – or do I need someone to help me manage where this business is going?

If you’ve simply got too many tasks, assistance might be exactly what you need.

But if you’re trying to scale, increase your team, introduce new revenue streams, improve your operations or build a clinic that doesn’t need you involved in absolutely everything, it may be time to consider management-level support.

We’ve also created a Business Growth Audit to help clinic owners identify where their business is currently strong, where they’re becoming the bottleneck and what operational support could help them move forward.

The audit is currently free to complete (usually worth £190) and gives us the opportunity to assess whether Fractional Business Management is actually the right fit for your clinic.
